If you play WoW, and have an Alliance character, you may end up in Ashenvale in your 20s.  If so, you might take a quest called "The Howling Vale", which tells you to "Go to the Howling Vale and study the Tome of Mel'Thandris, then return to Sentinel Melyria Frostshadow at the Shrine of Aessina."  This is all fine, except that, if you're like me, you probably didn't notice the name of the NPC who gave you the quest1.  So when the time comes to turn it in, off you go to the Shrine of Aessina.  Melyria Frostshadow is not there; she's still in Forest Song where you got the quest from her in the first place.  Signe carried that quest around, complete, until she happened to go back to Forest Song for something else and I saw the turnin dot on her minimap.

Also, if you're interested in quests where you get public acclaim, there's apparently one that starts from a corpse in the river beneath the Thandol Span (the bridge between the Wetlands and the Arathi Highlands).  I haven't checked it out myself, so I have no idea how easy the corpse is to find.
